Ohio Golden Buckeye Card Applications
The Clermont County Public Library has applications for the Ohio Golden Buckeye Card.
All Ohioans age 60 or better, as well as adults age 18-59 who have disabilities as defined by Social Security, are eligible for a free Golden Buckeye Card.

Do you have a Golden Buckeye Card, but have been told the card expired?
Do not apply for a new card if you already have a card.
Golden Buckeye cards do not expire. Follow the procedure shown below if you have been told that your card has expired and you are unable to receive your prescription discounts:
- To receive your prescription discount, you MUST call toll-free 1-866-301-6446 and inform the representative you have a Goldenbuckeye card but were informed the card had expired and you were denied the prescription discount.
- The representative will add you to the new computer system for RX discounts and you will be issued an ID number to use for prescription discounts.
