Ohio Golden Buckeye Card Applications

The Clermont County Public Library has applications for the Ohio Golden Buckeye Card.

All Ohioans age 60 or better, as well as adults age 18-59 who have disabilities as defined by Social Security, are eligible for a free Golden Buckeye Card.

Visit the Ohio Department of Aging website for information on the benefits of having a Golden Buckeye card.

Do you have a Golden Buckeye Card, but have been told the card expired?

Do Not Apply For A New Card If You Already Have a Card

GOLDEN BUCKEYE CARDS DO NOT EXPIRE. Follow the procedure shown below if you have been told that your card has expired and you are unable to receive your prescription discounts:

  • To receive your prescription discount, you MUST call toll-free 1-866-301-6446 and inform the representative you have a Goldenbuckeye card but were informed the card had expired and you were denied the prescription discount.
  • The representative will add you to the new computer system for RX discounts and you will be issued an ID number to use for prescription discounts.
